So it looks like the Engine mounts need to be replaced.

Need help with part numbers.

My understanding is that there are 3 Engine mounts?

Front
Front Right
Rear

Can someone confirm if the below part # are correct?

Front (Anchor Part #3131)
Front Right (Anchor Part #3130)
Rear (Anchor Part #3145)

What is the Transmission Mount Part #? Is it the same as the rear engine mount?

Does anyone have a write up or step/instructions on how to change?
 
I'm curious--why do you think the engine mounts need changing? What are your symptoms?

The engine is shaking when shifting from Park-Drive-Reverse

Started getting lots of clunking noises. Sounds almost metal to metal.

I have changed the following within the last 2 years:
Both Lower Control Arms to Moog
Sway Bar bushings to Moog
Sway Bar End Links (replaced twice)
Outer Tie rods to Moog
Strut to Monroe + Strut Mount to Moog (4 months ago).

The severe clunk started 3 days ago.
I confirmed nothing is loose including Exhaust. There is very slight play on the inner tie rods so I'll be changing to Moog also.

Figure I'll try the Motor mounts as there is quite the movement when changing from P-R-N-D

Anything else that I should look at?

How much movement are you getting on your engine, I did this test with vehicle in drive and revving it up a bit. I thought there was lots of movement but turns out it seems to be normal.

Punch your make/model/engine size in here http://www.showmetheparts.com/anchor/and double-check. Mounts aren't difficult to replace but ideally you'll need FSM for procedure. Rear mount is also the trans mount IIRC.

Jack, the mounts can tear and compress over time, changing them out makes a huge difference in NVH, especially idling in D on a CVT.
 
Also, is Anchor a good Brand?
In a word NO.

I used Anchor mounts (engine and trans) on my 95 Cherokee (XJ) and one of them (engine driver side) lasted less than a year, the other 2 failed after 2-3 years. I'd recommend OE mounts you should get another 10 years out of those (unless you are planning on selling the vehicle soon, then it might make sense to use cheap mounts).
 
Are the insulators the only part of the mounts that need replacing? I have a 2013 Patriot 4x4 2.4 that was in a front end accident. The motor vibrates so bad the hood shakes. Is changing the mounts a hard job? Are there and tips that will make the job easier? I check for OE parts and see that there a 4 mounts. Should I replace all 4 mounts?
